Description: I, the undersigned, GEORGE HENRY BROOKE , BOULTBEE-WHALL, heretofore called and known by the name of George Henry Brooke Bailey Whall, of The Rectory, Montgomery, in the county of Montgomery, and of Kalutara, Ceylon, Rubber Planter, hereby give public notice, that on the twentieth day of August, one thousand nine hundred and thirteen, I, on behalf of myself and my heirs and issue lawfully begotten, formally and absolutely renounced and abandoned the use of my Christian name of Bailey and my surname of Whall, and then assumed and adopted and determined thenceforth on all occasions whatsoever, to use and subscribe the surname of Boultbee-Whall, instead of the said surname of Whall
